Few points to make:
The two incidents, obviously involving two different vessels - one is white decked while the other is a classic looking dark timber decked boat - bearing the same name must be a freak occurrence. (better look out if your yacht's name is the same, it might be endemic

)
Why was the raft launched in the first place? There is a good size inflatable floating around.
Why was the raft launched on the windward side? Wonder how the sharp edges on the bow did not hole the raft being tossed around by the waves like it was.
Why was the raft not double tethered?
While the young chap was wearing a MaeWest type "lifesaver" on the top of his backpack the old chap only had a murderous backpack on his person. Had he fallen into the water while struggling with the waves getting on the raft, his backpack would have been his death.
I hope, for his sake, he had a valid insurance.
