I dont foil downwind, but got my share of high speed wipeouts (surf, SUP and foil), and I dont think head slams are an issue per se... except for eardrum rupture if you fall flat on your ear.
It happened to me (and my doctor... first day of his mentawai bodyboard trip ) and since then I always wear custom earplugs with a wide external part to protect from ear slams. I would recommend always wearing an ear protection while foiling (ear plug, helmet, hood, hat with ear flaps, headband, ...). Losing your balance to the point of not being able to stand up is something I would not like to happen on a downwind run. Plus eardrums never repair completely: they are made of 2 skin layers, and only one heals.
