Its expensive once there paying through someone like Avis or Budget. Has anyone got it covered before they go on their travel insurance or normal car insurance ? This hopefully will be a cheaper option. I checked one travel policy, but it only covers the excess, assuming you take out the cover with the rental company ?
Anyone got cheap cover ?

I can't remember the exact details but I have a Visa gold card and if I buy the airfares and hotel with my card I get Car and medical insurance free, once I'm outside Australia. It's worth looking at as the card only costs $70 a year.

We have been to maui twice, first time we went through a big car hire place, Alamo, we had a really nice brand new Chrysler Voyager kind of thing, we were jet lagged and fatigued, ended up getting flogged with about $600 additional insurances, this time round we hired a car through a cheaper car hire palce, Al Wests Car hire, we got travel insurance that covered up to $5000 of damage to the car, which we thought was enough, baring a catastrophe.
If you want piece of mind and want to be fully covered, pay for all the insurance you can get, so if you write off the car, you simply give them back the keys.
We were covered through Cover More for our insurance here.

Would that be enough insurance? I would have thought the main worry would be if you ran someone over or had an accident and you were sued a few million dollars.

If you're going to Maui use Word of Mouth car rentals. They buy all the used cars from mainstream rentals and rent them for half the price. They say don't bother with excess insurance on the car as they're only worth about $2k, so it's worth the risk. However there basic insurance coveres other parties.
