^^^ I actually lanced one exactly like that on my old man's back many years ago - with about the same amount of muck coming out!! And the girl's right - it STINKS like nothing else - washed my hands in Detoll twice afterwards and could still smell it

This is the worst thing I have seen - worse than mr hands, BME pain olympics, 2girls/guys one cup etc. and I saw it on TV!
Its disgusting I think that once you have been there you seem to appreciate the times when you are healthy much more. Mine is starting to disperse with the help of antibiotic long baths and prescription codine.
Probably with reef cuts i seen a similar thing with an arm that was cut open (bit of coral was left in wound) blew up like a baloon . if it gets to badly infected dont squeeze it and spread it . Get antibiotics or get it cut opened and drained accordingly with your gp/doctor. oR ask Kremlin
Scrub with nail brush or toothbrush... This is very sore but very important. You must do this till the bleeding starts Now pour metho and then vinegar onto wound (this is same as aqua ear) Metho dry wound, vinegar kill reef bacteria
Leave wound for airing... no pick of skin or scab and NO scratching of him

It is a Pilonidal Sinus, I had one similar. They are quite common mostly among people who drive for a living ie truckies, taxi drivers etc. and is basically caused by an ingrown hair that has become infected.
You need to see some one and if it has gotten to the stage of causing discomfort as you describe, then it sounds similar to mine. I was climbing and fell from a wall swinging back onto the wall which hit the sinus and burst it internally (No seepage). This caused Septacemia and I was admitted to hospital for a routine drainage. A bood test and temp check confirmed septacemia and they admitted me for surgery that night to remove it.
They needed to remove all effected tissue etc from the area and ended up making an incision 5 inches long and almost 3 inches wide. They removed all the meat from just below my tailbone to a depth of around 1.5 inches. Obviously with a cavernous wound like that, it wasn't possible to suture it closed and so it was left open and packed with saline drenched gauze bandage everyday for 8 weeks by a home nurse.
My case was quite extreme, to the point where my doctor was bringing medical students in to see the work he'd done. I knew it was bad when I saw the expression on their faces when the bandages were removed. The story goes on but the summary is to go see a physician to have it removed before it becomes a major issue.
It's not a big deal if caught early but can be a pain in the arse (pun intended) if let go to the lengths of my experience.
Good luck with it, most likely it will be a day procedure in and out.
